A diverse opportunity for an experienced clinician to join this federal government department on an initial 12-month assignment with flexibility to work from Mt Pleasant or Rockingham office. This is a unique opportunity to partner with key stakeholders including the Community and Peer Advisors to ensure best practice clinical care is being provided to the veteran community and their families.

Your new role
As the Clinical Care Coordinator you are responsible for complex case management and coordination of services. Reporting to the Assistant Director Community and Peer Program the position involves the development and maintenance of internal relationships, as well as external relationships with key community services stakeholders. Key responsibilities include:

• Work closely with Community and Peer Advisors (veterans with lived experience) with direct involvement in the community to support and provide clinical consultation and measure the impact of service delivery
• Direct provision of clinical services
• Intake, assessment and referrals
• Complex case management and case coordination
• Maintain accurate clinical records and prepare reports
• Liaise with external service providers
• Participate in community development and service promotion activities
• Contribute to the development of Open Arms programs

What you'll need to succeed
Key requirements include:

• Qualifications as a registered Psychologist / Mental Health Occupational Therapist / Mental Health Registered Nurse with full AHPRA registration, OR Qualification as a Social Worker eligible for membership of the Australian Association Social Workers (AASW) and be eligible to apply for Mental Health accreditation.
• Proven experience in clinical and complex case management
• Strong report writing and admin skills
• Ability to work in a fast paced environment
• WWCC
• National Police clearance

What you'll get in return
This is an APS6 government assignment with an initial 12 month time frame. The casual hourly rate on offer is between $55-$65 p.h + Super dependent on experience. Flexibility to work from home is on offer as well as flexible start and finish times.
