A Mascot man and a Blackheath woman were involved in a head-on car crash at Medlow Bath on June 20.
Detective Acting Inspector Brendan Bayliss from Blue Mountains Police said the man, 33, was westbound on the highway about 11.30am when he travelled into the eastbound lane and hit a car driven by the Blackheath woman, aged 73.
The man was trapped in his vehicle due to his injuries, and was removed by paramedics, before being flown to Liverpool Hospital with internal injuries.
The woman was uninjured, but both vehicles were damaged substantially and were towed from the scene.
"At this stage police are yet to speak with the 33-year-old male driver, and the cause of the accident is still under investigation," Det Bayliss said.
