The general manager of Blue Mountains Anzac Memorial Hospital has resigned.
Andrea Williams, who took sudden personal leave earlier this month, has decided she will not return and will instead retire.
Mrs Williams has spent 48 years working in health care. She began her career as a nurse and joined Blue Mountains hospital in 2002.
She has been the hospital's general manager for the past 13 years. Her resignation is effective from July 19.
The chief executive of Nepean Blue Mountains Local Health District, Kay Hyman, said in a statement: "I acknowledge Andrea's long service and dedication to caring for the health of the community and wish her all the very best for the future.
"Karen Arblaster, who is currently acting general manager, will continue in the role while recruitment occurs."
