Mr O'Toole will stand as an Independent in Ward 4 in the December 4 elections, with Blaxland resident Andrew Curnow as number two on his ticket.
Mr O'Toole was first elected to council for Ward 4 in 1987, before being elected deputy mayor the following year and mayor in 1991.
He finished his council term in 1995.
"I know the Mountains and I know local government. I believe we need better representation for Ward 4," said the Blaxland resident.
"Council is too focused on the Upper Mountains and tourism. It is the people of Ward 4 who carry the burden and receive little in return."
Andrew Curnow also targeted perceived neglect for the Blaxland area.
"I taught at Blaxland High School for nearly 30 years and have been heavily involved in rugby and cricket coaching, both at a school level and in the wider community," he said.
"It seems clear to me that when it comes to council funding the village of Blaxland does not get the attention it deserves. We need to be more active.
"The Lower Mountains has a history of successful community involvement to maintain village character. Mountains Against Flats in Area stopped overdevelopment in Glenbrook and Blaxland. Save Our Street movement stopped the destruction of an historic house in View Street and resident action stopped the construction of huge power poles in Rickard Road."
Both candidates have campaigned for the restoration of Blaxland War Memorial Park on the Great Western Highway near Wilson Way.
